Services Description

Hope Lane Disability Services is a registered NDIS provider offering person-centred, flexible, and culturally responsive supports to participants across Sydney and surrounding regions.

We are committed to empowering individuals to build independence, strengthen daily living skills, and achieve meaningful personal goals through tailored supports that reflect each participant’s unique needs, preferences, and aspirations.

Our services include ;

  • Specialist Support Coordination (Levels 1, 2 & 3),
    Where we assist participants in understanding and implementing their NDIS plans, navigating complex support needs, and connecting with appropriate providers.

 

  • Support Coordination that focuses on coordinating services, strengthening informal and community supports, and ensuring funding is used effectively.

 

  • Daily living support, including assistance with personal care, meal preparation, household tasks, and routine development, always with an emphasis on building independence and skill development.

 

  • Capacity building supports that develop life skills such as communication, social participation, budgeting, organisation, and community access. Our personal care and in-home supports are delivered respectfully and consistently, prioritising dignity, cultural sensitivity, and participant comfort.

We prioritise participant voice, choice, and control in every aspect of support, ensuring services are shaped around individual goals.

Our team has expertise across all levels of Support Coordination, including complex case management, crisis navigation, and multi-agency collaboration, allowing us to support participants with more complex needs effectively.

We also take an active advocacy approach, working to ensure participants receive fair access to services and appropriate use of their NDIS funding. Our practice is culturally responsive and inclusive, supporting people from diverse backgrounds with respect and understanding.

Capacity building is central to everything we do, with a focus on reducing long-term dependency by strengthening independence and everyday living skills. We remain flexible and responsive to changing needs, ensuring continuity of care even in complex or urgent situations, and we work collaboratively with families, support networks, LACs, allied health professionals, and other service providers to ensure coordinated and effective support delivery.

Description of who I am looking to connect with

We are looking to connect with individuals and organisations across the disability, health, and community services sectors who may benefit from or collaborate with Hope Lane Disability Services.

This includes NDIS participants seeking Support Coordination, Specialist Support Coordination, daily living support, or capacity building services, as well as families and carers who require guidance in navigating and understanding the NDIS system.

We are also looking to connect with Local Area Coordinators (LACs), hospital social workers, discharge planners, and allied health professionals such as occupational therapists, psychologists, speech therapists, and physiotherapists who may have participants needing coordinated or complex support.

 

We are further seeking to build relationships with support workers, case managers, community organisations, disability service providers, mental health services, and non-profit organisations who share a commitment to person-centred, strengths-based practice.

Our aim is to collaborate with professionals and services who value advocacy, capacity building, and holistic support, and who are interested in working together to improve outcomes and continuity of care for people with disability.

Lived Experience

I have grown up closely alongside my sister, who lives with Down syndrome, and that experience has had a profound impact on my perspective and the path I chose in life. Being part of her daily life gave me a deeper understanding of the strengths, challenges, and barriers that people with disability and their families can face, as well as the importance of dignity, inclusion, and genuine support that goes beyond just services.

This personal experience is what led me to study social work and ultimately step into the disability and community services sector. It shaped my commitment to making sure people are not only supported, but truly heard, respected, and empowered to make their own choices. It also strengthened my passion for advocacy, capacity building, and creating systems of support that are more accessible, compassionate, and person-centred.

 

For me, this work is not just professional, it is personal, and it continues to be driven by the belief that every person deserves the opportunity to live a meaningful and independent life with the right supports in place.
