When accessing the NDIS, participant assessment and the creation of a personalised Support Plan are two crucial steps for ensuring effective and meaningful outcomes.

What Is an NDIS Participant Assessment?

An NDIS participant assessment is a comprehensive process that evaluates an individual’s functional abilities and support needs. Conducted by qualified allied health professionals, this assessment covers key areas such as:

  • Communication
  • Mobility
  • Self-care and self-management
  • Learning capabilities
  • Social interaction

The goal is to understand how a participant’s disability affects daily life. Assessors consider both clinical evidence and the participant’s lived experience, ensuring the process is person-centred and holistic. These assessments are provided free of charge to participants, removing financial barriers to access.

Support Plan Development

Following assessment, a tailored NDIS Support Plan is developed collaboratively between the participant, their support network, and an NDIS planner or Local Area Coordinator. The process involves:

  • Setting specific, meaningful goals
  • Identifying strengths, needs, and personal preferences
  • Determining the types of supports and services required (e.g., therapies, in-home care, equipment)
  • Including risk assessments and strategies for risk mitigation

Support plans are reviewed regularly to ensure they remain relevant as needs and circumstances evolve. They are documented using official NDIS forms and templates to guide participants in maximising their funding to achieve greater independence, social participation, education, and employment opportunities.

Why It Matters

Not only can a good assessment save you time in administration and compliance areas, but it will also support you to deliver a safer, more on-point/relevant, and responsive service to your customers.
